This form is to authorize SoFLoGuitars to charge your credit card listed above for monthly guitar lessons on the 14^th of each month. Information on this form is only accessible by the owner and lesson manager of this company. In five years of giving lessons, we have never had a single incident regarding our customer’s confidential information.

The amount of this charge will vary depending on how many weeks are in the month for the day you take the lesson. You are charged for every week in the month. It will be $120, $160, or $200 per month. You are not charged for holidays that we do not teach. Lessons are not conducted on, Memorial Day, July 4^th, Labor Day, Thanksgiving Day, Christmas Day, and New Years Day.

I understand that I must notify SoFLoGuitars.com before the 14^th of any given month if I do not want my credit card charged for the next month’s lessons. We do not offer refunds for lessons you do not use, only store credit or lesson credit.
